Do i need to contact the specific school for the format required for medical fitness certificate or its a standard format for all the schools.

Can someone please share the format of medical fitness certificate required for school admission.

Do we need to get a print of the same on letter head of hospital or we just need signature from the hospital/doctor on a simple printout?
