the documents stated under form no 2.1,2.2,2.3 are to be in that form only or vaccination card and fitness certificate provided by our child doctor will be accepted or not and

It should be accepted.

Also, I think all these documents are to be kept ready and produced only during Pre-admission, when the schools checks the documents. You can't so many copies of these documents to be submitted with admission form.
