@ Mahesh, you can get it from any MBBS doctor, even R D Rajpal will ask for photo copy of complete vaccination chart of kid.

@ Mahesh, R D Rajpal Dwarka is providing forms at school and you can fill it and submit it there only.. Hey can you share you views about this school. Is it a good school?

@ Lathaa / All - I need some inputs about Modern convent and ITL public schools in Dwarka. I have heard mixed reviews about these schools, but their web sites and results look really impressive. Can you please help with some feedback ?

Plez help..... r d rajpal school dwarka website is not showing any form. I got the pdf form from the link given in admission nursery website. But The instructions in the pdf form shows that i have to fill it online and then submit the printout in school. Plez suggest what to do now???

If you have the reg number, then that is all that matters
