I went to Mamta Modern school and got the form. I am surprised to see that they have asked for the declaration that whatsoever loss happens to your ward (accident, injury, loss) in school, during any funtion at school, during any activity at school, during going to school or coming from school, school authority is not responsible. (Please have a look at attached scanned page 2 of the school form for exact wordings)
