Please help me in my query......

I have a birth certificate of my boy child made, but it is not having his name included in it....He is seeking admission this year ,,,,just wanted help about how to go about this inclusion procedure....please advice.....

Comment by shweta on January 6, 2014 at 8:40pm: Hi Anjali...you need to contact the hospital where your child was born if they are of any help..or go to the MCD office(address which is mentioned in your original birth certificate) and complete the formalities like affidavit n all. I got it done through the hospital only. but you need to hurry up because it might take 15-20 days which you don't have as the forms are started from 15 jan.
