ST THOMAS FORMS FROM 7 TH OCT DETAILS AS UNDER

Eligibility Criteria :- Children who are born between 1st April, 2006 and 31st March, 2007. Preference will be given to the children who live near the school (3 km ) and whose real brother or sister is studying in the school.
Application forms can be downloaded from school’s website www.stthomasind.com from 7th October to 14th October 2009.
Dates for submitting the duly filled application form at school counter :- 20th October to 22nd October 2009 between 8.30 to 11.30 am only.
List of eligible candidates will be displayed on the school notice board on 7th November 2009.
The Processing Fees of Rs.100 /- (Non-refundable) is to be paid only in cash along with duly filled forms.
Information filled by parents should be correct ,if the information is found to be incorrect or false, the ward shall be automatically debarred from selection/admission process without any further correspondence in this regard
Attested photocopies of the Documents to be submitted along with the application form:-


Municipal Birth Certificate
Proof of residence such as ration card, electricity bill, passport, voter’s ID etc.(any one)
In case of Christian , Baptism Certificate is mandatory
Orthodox Christian applicant shall submit the certificate from Vicar/Priest along with Baptism Certificate.
SC/ST/OBC Certificate (if applicable) to be submitted

Original Documents required at the time of Admission for verification.
Original Birth Certificate issued by MCD/Tehsil/Gram Panchayat only.
Original Residence Proof.
Family Photograph.
Medical fitness certificate.
