* This affidavit should be on a non-judicial stamp paper of  Rs. 10/- and should be duly attested by Notary Public.

 

I ________________________ S/o_______________________________

R/o__________________________________________________________

do hereby solemnly affirm and declare as under :

1. That I am applying for admission of my

daughter named _______________________________ for Pre-School/ Nursery

(Nursery) (2018-2019).

2. That I declare that __________________ is our first born Girl child 

and she does not have any elder sibling.

 

DEPONENT

If it's a self affidavit different format

if a girl has elder brother then also we need to fill this form????

School specific if they ask for this girl child affidavit

Hi Parents

Some of the Schools have their own specific formats for First Child & Girl Child
Do we have to submit original affidavit to each school at the time of form submission? I have twins, both girls.

Some schools accept zerox at the time of form submission ( school specific)
