Hi,

There is no prescribed format given by Ramjas School.

You can visit your nearest Notary office and ask them to prepare First Child affidavit, they have this set template where they just change the details and you will be done in like 5 mins. 

Stamp Duty Amount: Rs.10/-

Total Affidavit Cost: Rs.80/-

Also, Ramjas School (Non-Day Boarding) requires this First Child Affidavit in Original while submitting the form. So it is advisable you get 2-3 copies made in case you require it for other schools too.
