About respective Affidavits required for nursery admissions

Hi,
I have a query about the affidavits like first child affidavit etc.required for admissions...would these be school specific and need to have the school's name mentioned or is it a generic one which we can get done in advance and keep it and get it photocopied for whichever school it is required for? Kindly address this urgently pls...TIA...

Few are schools specific but general format and at the time of admission original affidavit has to be given

Thank you Sakshi...Would like to check about the Immunization Certificate and Medical History of the child...This needs to be on separate plain papers in this format or can we get the immunization record photocopied and attested by the Pediatrician of the child and the medical certificate on the letter head of the doctor concerned?

For immunization, medical (immunization chart) records will do and Medical Certificate must be issued by a Registered Medical Practitioner with qualification of MBBS or above on his/her letter head .
