What all documents are required along with filled up form.

What are acceptable address proofs and do the documents need to be attested by gazetted officers.

I have an allotment letter for my flat where i am yet to shift. Can it be used as address proof.

New schools might accept it but not old established ones (school specific)

Last year Govt allowed self attested doc but still double check with schools
