How to obtain immunisation certificate for nursery admission?

You need a copy of Immunization chart of your ward ( As you can get zerox also if required by school at the time of applying) and medical certificate from Family doctor.

Any doctor can give this certificate? Or does he have to be certified?

School specific ( Most of schools accept by Pvt Doctor/Hospital) unless mentioned by school to get it from Govt hospital.

from your paediatrician who has vaccinated your child. they keep record of all immunisations given..you can ask for a copy.
