Comment by Aditya Agarwal on September 6, 2013 at 2:44pm: Yes Rajeev. You have to take a print out and submit the form by 8th Sep. I will do it tomorrow as 8th is Sunday and I am not sure if they will be open on Sunday.

For form filling it's quite cryptic, just see you are not exceeding word length and check out all the necessary details if they are filled properly.

Comment by Aditya Agarwal on September 6, 2013 at 2:39pm: Finally able to fill the form and submit it successfully. As said earlier length should not exceed 250 words. In fact I kept it atmost 2 lines. The form print which came missed my child's pic but I called them and got to know it's not mandatory and the form needs to be submitted at school with BC, Address Proof and Photographs of child and parent.

Comment by Purva sharma on September 6, 2013 at 2:35pm: Just attempt it, after deleting the cookies, History and then fill it fresh. Fill it from your personal LAP top. Last year even we faced so much problem in filling it. I and other parents also went to the school reception and they genuinely considered it and resolved the matter.In our case the number of words were exceeding, that's why the form could not be submitted. Everybody was facing the some or the other same problem.
