PLS TAKE 2 VERSIONS OF THE FORM

1. (DOWNLOADED after submission) and

2. Updated (Hand correction done)

3. Application stating reason for change and list the changes.

While you go to school for form submission alongwith Hard Copy, try submitting updated form, if school does not accept, attach your written application which reflects the changes/ updates/misses.. and your problem should be solved.
