Documents preparation for admission
Hi,
Kindly guide me on following documents required for nursery admissions -
- Affidavit in case of first-born child/ girl child (Affidavit has to be on Rs. 100 stamp and has to be notarized?)
- Immunization chart of the child; medical certificate (Will vaccination chart suffice or a separate medical certificate from te doctor would be required?)

Few schools ask for immunisation chart i.e. vaccination chart and most schools ask for medical fitness certificate only.

Mam could you please let us know if all these documents need to be submitted along with the form in each school or at the time of verification if the child is shortlisted. Also does an original Medical certificate/ a girl child ( not first) affidavit / distance affidavit needs to be submitted during putting in the application.
