Medical fitness certificate format

Filling in indraprastha form. They need inoculation / medical fitness certificate.
Can anyone please help me with the format.
Or can i just attach vaccination card copy?

Yes

vaccination card copy plus letter from your family doctor .he will make it and would be knowing as it is generic
