Birth Registration Certificate Number in the form mean registration no. or (Avedan kramank ) or Praman patra kramank in the birth certificate? Please guide

there are 3 no.s mentioned in the birth certificate,

1.registration no

2.Avedan kramank

3.Praman patra kramank

which of these is the Birth Registration Certificate Number? :(

Hindi word for Certificate is "praman patra" so I would say the 3rd one...

Guys I believe only DPS Noida opened registration in month of June other will open in month of July or August onwards. Right??
