There is no prescribed format for it.
Just visit any medical practitioner and ask for a medical fitness certificate for school admissions. And they will eagerly help u.
Just for your reference, medical certificate should have *Childs' name, fathers'/mothers' name, permanent address, and describing that ward is not suffering from any communicable diseases' and fit for any outdoor activities. And I certify his/her vaccination record are updated.
