Are there different parameters for type of medical certificate required to different school?

Wanted to know If the medical fitness certificate based on the vaccination card would be sufficient..or there are any different parameters set by schools and they have their own medical chart ?

 It's just a formality. Take it from your family pediatrician, he will have the format.

Medical Certificate can be made from any doctor near you on its letter head. Every doctors know the format. Its just a information form the doctor that child is fit and doesnt suffer from any disease. but take child with you.
