HI Saakshi, 

I have also got call from DPS Mathura Road for verification, do we have to carry medical certificate now or at the time of admission. The guidelines are not clear.

Please suggest.

hi priyanka, i think you should carry doctor's medical certificate to be on the safer side. we need it anyhow. when do you have to go?

Priyanka, for which class doc verification is being called ...and how many point u have... I too have been called for doc verification for class 1 ( 70 points )

Yes medical form as per prescribed format and also we need immunization card copy too.

Please let me know what all we need to carry:

1. Online application form

2. Address proof

3. Birth Certificate

4. Immunization card

Do we also need to carry Demand Draft?

No need of demand draft at time of verification 

Take also copy of  adhaar card of child and if you have mentioned adhaars of parents too then take their copies too.

Also take medical form signed by your doctor as given in their guidelines along with immunization card

Birth certificate n address proof copies of both parents...

my husband lives in another city. is my adhaar sufficient for address proof
