Queries for Nursery Admission for 2019-2020 batch

I will be grateful if anyone can help me with below doubts.

1. We stay in a rented accommodation so can we give Rent agreement as Address Proof?

2. In case rent agreement not allowed... Is it okay if only child's father can show Aadhar Card with current address or do we need to produce address proof for Mother also?

3. Do we need to submit any address proof for Child also?

4. We need to submit xerox copies of all documents with Admission forms. Original documents are to be presented at the time of admission... Right?

6. All schools will provide forms online or some can ask us to come collect the form personally?

7. How will we get to know of fee structure of every school we apply? Like do we need to visit school first to know their fee structure or we will get to know about it after filling the form?

8. There is a mismatch of Mother's name of child's Birth Certificate and Mother's ID proof. My daughter name is **** VOHRA and her father (xyz VOHRA) has all documents (ID and Address proofs) rightly available but Mother has all documents available in her Maiden's name (abc MEHRA). But my name on her Birth certificate is abc VOHRA. Can this cause a problem?

11. is it mandatory for Mother to show ID proof of same name which is mentioned on Child's Birth Certificate or Surname can vary and won't cause a problem?

12. Is it mandatory to submit an affidavit (First Child, girl Child) duly signed by SDM only or we can get this done from a local notary on stamp paper.

Some of indicative documents which can be considered as proof of residence of
parents/child:
(a) Ration Card/Smart Card issued in the name of parents (Mother/Father having
name of child).
(b) Domicile certificate of child or of his/her parents.
(c) Voter I-Card (EPIC) of any of the parents.
(d) Electricity bill/MTNL telephone bill/Water bill/Passport in the name of any
of the parents or child.
(e) Aadhaar Card/UID card issued in the name of any of the parents

Address proof of any one parent can do. So the proof of child's father can do and mother's proof not necessary. Your 4th point your mentioned is right. It depends on schools few do online admission process. else form will be available in site and you have to download and submit in school, or both issuance and submission may be done in school. Fee structure few schools display or after selection schools may issue the fee structure. Regarding your name in birth certificate does not affect the admission process. Affidavit is school specific and many schools ask for self declaration from parents. Affidavit from SDM only necessary if schools specify it. DPS generally ask 2 address proofs.
