Indian School Admission form query

HI

I am filling up Indian school for my nursery admission 19-2020 whereas they have mentioned "Joint Declaration of First child by both parents"  and 'medical Fitness certificate of the child"

Do we need to create affivadit for this declaration?

Kindly guide.

I went to notary today to to ask for Summer Fields school (who also need a first child affidavit) there person at notary told me that joint declaration means that You need to have a notorious affidavit having both your and your's spouse sign.
