Hi All,

Can anyone tell me what is required for Medical Officer's Report for admission in DPS Sec45 Gurgaon Nursery class?

Is it required at the time of admission fee deposit?

Is there any format for this report?

This is just some informartion written by any doctor on his letter head with his sign and seal mentioning:

height, weight, no allergy, vaccination is upto date and child is medically fit as of now.

That's it !

The medical card..where all the vaccinations are reported..
