Birla Vidya Niketan is asking proof of first child as discharge slip from the hospital child was born, Incase we don't have the discharge slip , can we submit an affidavit for the same.

A notary affidavit along with Doctor's certificate will do but enquire with the school since we don't want to take any chance on your child's career.
