Yes apart from birth certificate first child affidavit is required to claim the points allotted to first born child.

Affidavit is normally made by notary public. If schools asked for a self declaration then you can write by your own. Or else may be schools are accepting self attested photo copy of the original affidavit made. Just confirm with the school.
