What is the format for this ...   is it just on plain A4 paper or on govt approved notary??

Where do i get that proof and its format?

Can someone help me  on my doubt.I have one query once we get the application form we have to submit the application forms only na that time there is no need of documents.

Once we get the number in any school then only we have to submit the documents.
