Affidavit for First Child and /or Girl Child

Affidavit required by schools for First Child and/or Girl Child can be generic for all schools with whomsoever it may concern? or the affidavit has to carry the name of the respective school and we need to get seperate affidavits for each school customised?

Else we can get multiple similar generic affidavits made?

You can make a generic one. Make a school specific only if any school has mentioned in it's guidelines. Few schools may ask only a self declaration and it's school specific.

Declaration made by parents in a plain paper with attestation by parents. Refer the 1st format for self declaration by parents in http://www.admissionsnursery.com/page/documents-required-1

This information will be given in the school's form in criteria note.

From where do we get this Affidavit? Is it the Notary or any particular department?
