Father and Mother can fill up and sign as below format for a Joint declaration as the case may be

We  parents  __________name______________ (Father) and ________name _______________________

(mother) R/o__________________________________________________________

do hereby solemnly affirm and declare as under :

1. That we have for admission of my daughter /son named _______________________________ in Pre-School/ Nursery

(2015-2016).

2. That we declare that ____name______________ is our first born(Boy /Girl) child /

and he/she does not have any elder sibling.

Joint declaration can be done in a plain paper.
