1. Where do we get the format for various affiddavits that may be required like - First Child. I suppose for Girl Child ,no affidavit may be required.

For DOB proof you have to submit the birth certificate only.

my question is the same as mr anil Jhambs, is an affidavit necessary for girl child , and wr to obtain the format.
we are trying to get my daughter's name added to her birth certificate,can u please guide me as to what documents would be needed and where i can obtain the format for the affidavit.

Affidavit not necessary for girl child. For name inclusion an Affidavit on stamp paper of Rs.10/- duly attested by notary public having subject name inclusion of child is needed and if you visit MCD office for registration these notary public will be available just near to this desk. If you have the birth certicate and a copy it will be ok. Have residence proof even if it not required. The process will take 7 days time and always have some original copies of birth certificate which you can get there itself. 

Is Ration Card accepted as valid address proof?

2 Yes many school accept it (but some school don't accept it as the date is old in some cases )

@Aditi ,Most of the schools don't accept Ration card (except very few).

can Sibling cousins can be used for sibling category . please help

@Taimur ,Sibling means real brother or sister in same school only

@sakshi.thanx for the format , pl let me know if a can find the format the affidavit for adding the name to the birth certificate somewhere, we r really struggling with that , can't find a notary who knows anything about it.

That is available in MCD office only
