I guess we can add both the points in certificate so that there will not be any headache of re arranging the same for particular!

But doctors will write in their letter head and they know what to write no worries. Go to the nearest Primary Health centres or Govt Hospital.

has it to be from Govt Hospital only . 

cannot it be from the pediatrician

Pediatrician is also a registered doctor. So you can get from them. They will write in their letter head since their registration no. will be mentioned in it.  Every qualified doctor must register with the respective council like Medical Council of India & Central Council of Indian Medicine/Homeopathy/Dept. of  AYUSH,MOHFW, GOI.  You can get from private Doctor or Govt. doctor but  he/she doctor should be registered and have registration no. or id.

hi,

i have a certificate where on Dr. letterhead contains one DMC no...is this the only regd no or should i ask for again from the paediatrician.

Photo copy of the medical certificate to be submitted at the time of application,,, Submit the original to the school where you pay the admission fees finally...

Get it in writing that proper immunizations has been given and child is medically fit.

Can we submit photocopy of certificate? OR Do we have to bother doctor for all original certificates? Depends on no of forms we are filling.
