Schools asking for declaration girl child and first born how and where to get it from?

Hi admin.......

Maximum schools are asking to submit documents with the form one of those document is declaration of first born and girl child. could you please guide me from where and how to get that declaration.

You have to get from a notary public. Format is given but notary knows how to write it.  You can leave the school name in the format unless specified by the school.
