Hi,

Wanted to ask for medical ceritificate, do we need to make all the 3 certificates which you have mentioned in Documnents required?

MEDICAL HISTORY OF THE CHILD [Form 2.1]

MEDICAL FITNESS CERTIFICATE [Form 2.2]

IMMUNISATION CERTIFICATE [Form 2.3]

How should we do this?

Hi madhur, Which certiicates are you referring to? Have I missed something? To the best of my understanding, DOE has not released any certificate format for schools to use. The schools may ask for details in specific formats as per their requirement. That depends on schools and may vary from school to school.

Rakesh,

I am asking in general about the vaccination/medical certificate which we will require for school admissions. In list of documents mentioned in Document Required Tab on below link, they all are the documents format/affidavits which mostly school use.

So, for 1st, does that needs to be on some stamp paper?? or just the paper/letter from Doc would suffice?

Hi, will the admission schedule of the 285 schools for which the guidelines are awaited, be different from the other schools? Please clarify.
