If its not mentioned get it attest by a gazetted officer to ne on a safer side.

Please also let me know if there is any Format for Medical certificate which is required by Indian School.

For Indian - Medical Fitness Certificate of child from the practitioner looking after the progress of your child. So you should get from your pediatrician and they know what to write in his/her letter head.

As per Govt. order self attestation will do but school specific. Confirm with school.
