Hi Devika,

I have submitted lotus valley form twice and both the times window got disappeared after clicking on submit button. I used chrome and made sure to turn off the pop up blocker. Not sure what to do next. Has anyone also tried ?

Very basic interaction. Where you work, how much time you spend with kid, who all are in the family, expectations from the school etc. Kid was asked to recite any poem and identify some colors.
That's pretty much it.

I think so it is serial number wise. Mine was 0031.
