Registration to Pre-school (2023-24) General Category

Kindly read the instructions given below very carefully:

1) Minimum Age: 3 Years as on 31st March 2023. Upper Age Limit Less than 4 Years as on 31st March 2023
2) Fill the form in Capital letters only.
3) Online Registration will be done through school website – http://davkailashhills.com
4) Payment will be made ONLINE.
5) Keep your details of Debit Card/Credit Card/Net banking ready for online payment of Rs. 25/- as the registration amount
6) In case of single parent, the entire information of the parent is to be repeated in both the columns.
7) Fill up the form completely before taking a printout. In case of incomplete form/documents, the application will be deemed as rejected.
8) Print out of the filled registration form, duly signed by the parent, must be Deposited at the school fee counter , along with self-attested photocopies of the supporting documents.
9) Last date of online submission of Registration form for pre-school is 23-12-2022 on Website.
10) Transport facility is available on existing routes and stops.
11) After registering your email id, please wait for 5 minutes to receive the password.
12) If payment process is not completed successfully due to any reason, proceed again using the same e-mail Id & password.
13) Kindly keep the photographs in SOFT copy ready for uploading while filling the form. (Applicant’s, Father’s and Mother’s photograph). The size of the photograph should not exceed 200 KB and should not be less than 50KB.
 

Required Documents :

(i) Proof of Date of Birth : Birth certificate of child issued by MCD/NDMC/Municipal Corporation.
(ii) Proof of Address (Any 1) :

• • Ration card/Smart Card issued in the name of parents (having name of child).
  • Domicile certificate of child or his/ her parents.
  • voter I-card(EPIC) of any of the parents.
  • Electricity bill/MTNL telephone bill/water bill in the name of any of the parents.
  • Passport issued by Govt. of India in the name of any of the parents or child.
  • Aadhar Card/UID Card issued in the name of any of the parents
  • Passbook of running nationalised bank account having the photograph of the parents, Attested by the bank manager.

         Please note: Mobile Phone and internet bills will not be acceptable.
         Acknowledgement receipt of Aadhar card/UID card will not be acceptable.

(iii) Proof of alumni of DAV PUBLIC SCHOOL, KAILASH HILLS : (Passing Certificate).
(iv) Proof of Sibling : Latest fee bill / Report Card / Identity Card

(v) If the child born in April 2020 proof attached.

• The complete form should be dropped in the drop box, available at Gate No. 2 of Faith Academy School between 09:00 a.m. to 05:00 p.m. Last date of form submission is 23.12.2022, 05:00 p.m.  

The offline application will be filled in CAPITAL LETTERS.

SELECTION OF CANDIDATES FOR ADMISSION IN PRE-SCHOOL FOR ACADEMIC SESSION 2023-24

(i) School follows the 100-point scale system.

 (ii) Out of the total applications from the distance criteria range and Sibling Criteria admission would be first offered to the candidates attaining maximum points.

(iii) In General Category if there are more than one application attaining the similar number of points, then a draw of lot will be conducted to admit student against the available seat.

(iv) If vacancies remain unfilled after exhausting the applications from the first lot of maximum scorers, the applications from the next lot of maximum scorers shall be considered in the above manner.

AGE LIMIT

As per Directorate of Education (D.O.E) Order the child should be at least three (03) years old as on 31st March of the year for admission in Pre-school. However, the child must be less than 04 years old as on 31st March 2023.This includes all the children born between 01.04.2019 & 31.03.2020.

REGISTRATION

Standardized registration forms shall be available Online on the school website http://gdgoenkamodeltown.in/ and http://gdgoenkamodeltown.com/ on payment of Rs.25/- only.

The filled in forms should be deposited through the school website on all working days. The last date for deposit of application form for registration shall be 23^rd December’ 2022. (Friday)

   1. Self-attested copy of Birth Certificate.
   • Name of the child must be endorsed in the birth certificate duly counter signed by the Registrar of Birth.
   2. Proof of Residence required. (Self-Attested)
   • Voter Id (EPIC) of any of the parent.

   3. In case of Sibling.
   • Photocopy of the fee receipt of last quarter (paid).

   4. In case of First Born.
   • Self -Attested Declaration duly signed by both the parents.
   5. Copy of Aadhar Card of Parent. 
   • Duly Self Attested.
   6. Copy of Aadhar Card of the child applying for.
   • Duly Self Attested by both the parents.
   7. Original Medical Fitness Certificate.
   • A medical Fitness Certificate declaring that the ward is physically/mentally fit to join the School which should be duly signed by the Doctor along with the stamp and with his registration number.
