3. Sibling Proof (real brother or sister of the applicant only). Attested photocopy of latest fee
slip of real brother/sister studying in the school.

(CRPF CATEGORY)
1. Self Attested photocopy of Date of birth certificate of child issued by MCD/NDMC or
any other authorized body.
2. Latest passport size photograph of the child, pasted in the space given in the form.
3. Service certificate in original of either of the parents (Father/Mother) serving in CRPF
issued from CRPF with Rank, Force Number & place of posting clearly mentioned in the
Service certificate.
4. Attested photocopy of the I Card /Service Certificate, either of the parents
(Father/Mother) serving in CRPF.
5. If belongs to SC, ST, OBC-self attested photocopy of the certificate.
6. Sibling Proof – self Attested photocopy of latest fee slip of real brother/sisterr studying in
the school.
IMPORTANT INSTRUCTIONS.
 In case of incomplete form, incorrect/false information/document, the form is liable to be
rejected.
 Any other documents/certificates which the school requires for verification at the time of
submission/admission.
 Final admission is subject to the verification of all the documents in original. Only
submission/filling up the form does not guarantee admission
