 General Instructions:

2. The name of the applicant, parent, date of birth etc. should be as per official record.
3. It is mandatory to fll all felds which are marked in red. If any feld is not applicable to you,
please write NA (Not Applicable).
4. An incomplete admission/registration form will automatically be rejected.
5. After completing the form, click on the Submit button at the bottom of the page. You will
receive an auto-generated Acknowledgement Receipt with a reference number, USER ID
& PASSWORD for further correspondence.
6. You will receive SMS also as an acknowledgement with reference number, USER ID and
PASSWORD for further correspondence.
7. The USER ID & PASSWORD should be preserved to check the online admission status of
the applicant.
8. Take a print out of the form through Print Form option and Registration Receipt and
submit at the school’s reception along with registration fee (Rs. 25/-).
9. NOTE: Applicants must retain a copy of the printout of registration form and Registration
receipt for their record.
10. Submission of Registration form does not guarantee admission.

5. Important information regarding admission a)The child should be minimum three years old as on 31st March 2020 and as per the Directorate of Education norms i.e. less than 04 years   as on 31st March 2020. b) After filling the form online, HARD COPY/ PRINT OUT of the filled form is to be submitted at the school reception along with the self attested requisite documents on or before 27th December 2019 between 9.00  a.m to 12.00 noon on all working days. 6. The particulars of the child to be filled correctly and D.O.B must tally with the one recorded in the Date of Birth certificate. No change whatsoever will be made after the admission. 7. The management of the school reserves all rights of admission or rejection and is not bound to give reasons for admission or rejection of any particular candidate. 8. Submission of form does not mean grant of admission. 9. Incomplete registration forms or those with incorrect information will automatically stand cancelled. 10.The school does not accept any donation/capitation fee for admissions. Parents should beware of third party making false claims of procuring admission and collecting money on behalf of the school. If they enter into any transaction with such parties, they will be doing so at their own risk and the school shall not be responsible for it. The school will appreciate to know if there is any such person so that proper legal action can be taken against them. 11. Final admission is subject to the verification of the following documents in original. Photo
copies will not be accepted. Parents must bring the originals for verification purpose.
 Original Birth Certificates of the child issued by Municipal Corporation or equivalent
authority.
 Any two Proofs residential address :
 Ration Card issued in the name of parents(Mother/Father having name of the
child)
 Domicile Certificate of the child or of his/her parents.
 Voter I-Card (EPIC) of any of the parents.
 Latest Electricity bill/ MTNL telephone bill/ Water bill/ Passport in the name of
any of the parents or child.
 Aadhar card/ UID card issued in the name of any of the parents.
 Sale or Lease Deed in the name of any of the parents.
 In case of rental accommodation, valid Rent Agreement and an affidavit on
stamp paper is to be submitted. School reserves the right to verify the residential
address through home visits.
 Medical fitness certificate of the child issued by a Registered Medical Practitioner in
Delhi.
 In case of sibling applicants, the school Identity Card /Fee Bill of the sibling (Real
Brother / Sister) studying in J M International School, Dwarka.
G. CERTIFICATE FROM THE PARENT
1. I/ We hereby certify that the above information provided by me/ us is correct. I/We
understand that if the information is found to be incorrect or false at any point of
time, my/our ward will be automatically debarred from the selection/ admission
process and admission will be cancelled by the school without any correspondence in
this regard.
2. I / We also understand that the application / registration / wait-listing do not
guarantee admission to my ward. I / We accept the process of admission undertaken
by the school and I/we will abide by the decision taken by the school authorities.
3. I undertake to submit all the required documents in original for verification by the
school authorities and pay fees within the stipulated time period.
